Are you a dedicated Occupational Therapist looking for your next opportunity? Stokes Adult Social Care team is seeking a skilled professional to join their dynamic team. This role focuses on supporting adults to live independently and improve their quality of life through personalized care plans and interventions.Key Responsibilities:Conduct holistic assessments of service users’ needs.
Develop and implement tailored intervention plans.
Collaborate with multidisciplinary teams to deliver person-centered care.
Recommend and arrange appropriate equipment and adaptations.
Provide expert advice and guidance to service users and their families.
Requirements:HCPC registration as an Occupational Therapist.
Previous experience in adult social care or a similar setting.
Strong communication and interpersonal skills.
Ability to manage a diverse caseload effectively.
