Join us and contribute to making a positive impact on our community.The RoleAn exciting opportunity has arisen for 2 dedicated and experienced Social Worker’s to join our Complex Intervention and Treatment Team (CITT). CITT is an integrated team (North Somerset Council Adult Care and Avon & Wiltshire Mental Health Partnership NHS) responsible for the health and social care of older adults with mental health problems.The postholders will be based within the Adult Social Services and Housing Directorate/Avon and Wiltshire Partnership at North Somerset Council, working in the Mental Health Team, which is responsible for meeting the health and social care needs of people with a severe enduring mental illness. This involves the provision of advice and information, carrying out assessments of need, preparing Care plans, providing support and a range of personal social services to users and carers. AMHP’s within the Team fulfil the statutory functions of the Local Authority under the Mental Health Act 1983.Working with people with severe mental illness will involve supporting the most vulnerable members of the community, with a range of complex needs, requiring a highly skilled approach in assessing risk, treatment and need. The postholder will engage and work with service users and their carers to achieve the best possible outcome for their future quality of life.In this role, you will assess, within the relevant legislation, the risk , health and care needs of service users, including carers of individuals, who have a range of mental health problems, planning, implementing and evaluating appropriate action, negotiating for and co-ordinating the application of resources and/or if necessary, provision of service.If a qualified Approved Mental Health Professional (AMHP), to provide an Authority wide AMHealth Professional service under the Mental Health Act, for the Assessment of mentally ill service users. To ensure that there is no infringement of the Human Rights Act.Key Responsibilities:
